St. Anthony’s has partnered with Triton Healthcare to provide an array of comprehensive integrated rehabilitation programs to its residents. Program goals include restoration of patient functioning, improvement of quality of life, and the return of the least restrictive setting appropriate for the patient’s needs. Each of our Rehab to Home Rooms come beautifully furnished with cable ready TV’s and free Wi-Fi to comfort your loved one during this transitional time.
Our team will work with each resident to ensure a smooth transition from the hospital to our skilled nursing facility and from our facility to home. Skilled nursing care combined with rehabilitation services will work together to make their recovery successful. We will evaluate each patient’s needs to ensure they will be able to safely return home.
Our physical therapy program involves the evaluation and treatment of orthopedic, neuromuscular, and medical conditions. Our goals are to maximize each patient’s functional activity level, relieve pain, and reach their maximum potential. We emphasize family and patient education in the rehabilitative process.
Our occupational therapy program involves the evaluation and treatment of conditions interfering with the functional performance in persons impaired by physical illness, injury, cognitive disorders, or congenital disorders. Our focus is to achieve the optimum level of function for each resident.
Speech & Language Therapy
Our speech & language therapy program goals are to increase functional communication, cognitive skills and teach safe swallowing by introducing diet modifications and special feeding techniques. Our programs may include muscle retraining, compensatory strategies and the use of communication devices.

Our staff utilizes the skills and expertise of each discipline to plan, implement, and facilitate all pathways for returning an individual to their highest practicable physical, mental, and psychological functional level and wellbeing.
